Spring Break is almost here and before you know it we will be off for Summer.
A few reminders:
1. Please do not send your child to school with medicine. A parent or guardian needs to bring any medication for child to clinic and fill out "An Authorization To Give Medication Form". I do have one on my blog that you can click on and download from home. Please do not send child with cough drops unless the clinic has a permission slip on file signed by parent or guardian. You may stop by the clinic and get one.
2. Two weeks prior to school ending this year, I will be sending home letters to parents reminding you that if you have medicine for your child, you will have to pick it up prior to, or the last day of school. If medication is not picked up it will be discarded. No medication can remain in the clinic over summer break.
3. I will be also sending home forms to any parent whose child has medications for Asthma, Allergies, and Diabetes. Please have your doctor fill out doctor's forms and parent fill out parent forms and return at the beginning of the next school year. If your child is in 5th grade please return forms to the nurse at the school in which your child will be attending next school year.
